Volunteers List for 09-10

Now is the time to submit your Volunteer Application form for next season (Fall 2009-Spring 2010).  Please prepare your Volunteer Application form on-line and then submit it IN PERSON to the Child & Volunteer Protection Advocate (CVPA) in August, 2009 or by mail.

HOW TO Pre-REGISTER AS A VOLUNTEER ON LINE

To prepare your Volunteer Application form on-line, go to www.eayso.org Select “First Time” (if you have not previously registered) or “Log In” if you already have a password. Follow the instructions – we are Region 149. Fill out the on-line Volunteer Application form. Provide as much information as you can, including your email address and employment information. All fields with red asterisks are required data. When done, print out two (2) copies of the Pre-Registration form. You will notice that only a series of XXXXs appear in the Social Security box and the Driver’s License box. That information is in the AYSO database, protected by state-of-the-art encryption, but no person will be able to view that information. Now, you are almost finished.

Sign both copies of your printed form. The signed forms must be submitted in person, along with photo ID, to the Region 149 Child & Volunteer Protection Advocate (CVPA).

Volunteers have several opportunities to submit their registration forms to the CVPA during the player registration events in May, or they may do so at the Team Organization Meeting in August. If you missed the registration events in May and wish to avoid the crowds in August, you may mail (2) signed copies of your form, along with a Xerox copy of your driver’s license to: AYSO 149 CVPA, PO Box 516, Philomath, OR 97370. Mail-in forms must be received prior to August 1st.

Team uniforms & equipment are issued to the Coach at the annual Team Organization Meeting in August. All of the volunteers on your team filling the Coach, Assistant Coach, Team Parent and Referee positions must be registered with the CVPA before uniforms and equipment will be issued to the team.

You will not be “registered” as a volunteer until the CVPA enters your Pre-Registration form into the National AYSO database. At that time, new volunteers will be issued an AYSO ID number. If you are not registered, you are not eligible to participate in AYSO activities.

If you are unable to complete the online registration, you may obtain a blank form from the CVPA and submit a handwritten volunteer application form.

Why AYSO Volunteers Must Register

The national AYSO organization requires every volunteer who will be working directly with children to register each year. Those volunteers include all coaches, assistant coaches, referees, team parents, board members, and persons filling certain other positions. Registering accomplishes two things. First, it provides liability protection for the volunteer under the Volunteer Protection Act of 1997 for unintentional harm caused by the volunteer “if the volunteer was properly licensed, certified, or authorized by the appropriate authorities…..and acting within the scope of their responsibilities.” Second, it provides the information needed for AYSO to run criminal background checks to screen for persons convicted of sex crimes and certain other serious crimes as authorized by the National Child Protection Act of 1993.

If you are not registered with, and authorized by, the Region to be a volunteer, you are not protected by the Volunteer Protection Act, and you may be held personally liable for any mishaps which occur during your volunteer activities. The members of the Region 149 Board of Directors may also be held liable for failing to exercise “due diligence” in assuring that all volunteers are properly registered and trained. It is the duty of the Regional Child and Volunteer Protection Advocate (CVPA) to assure that every AYSO coach, assistant coach, referee and team parent submits a valid Volunteer Application form each year. The Region 149 year begins with the Fall season, so volunteer registration begins with player registration in May and is completed before play begins in September.

If Region 149 does not have a valid Volunteer Application Form from you for the Fall 2009 – Spring 2010 season, you will not be allowed to be a Coach or Referee or a Team Parent. VALID means that all information requested on the form has been provided by you (including Social Security number), and that 2 copies of the signed form have been submitted in to the CVPA with current photo ID. Failure or refusal to complete any portion of the form will result in rejection of your application and you will not be allowed to volunteer with AYSO.

SAFE HAVEN TRAINING

Every coach, assistant coach, and adult referee participating in AYSO must also complete Safe Haven training. You must attend Safe Haven training only once, NOT annually. Safe Haven training and certification are available online through the National AYSO website at www.aysotraining.org You will need your AYSO ID number to access the courses. If you have registered as a volunteer but do not know your AYSO ID number, you can obtain the number by sending an email request with your name and phone number to our CVPA at This e-mail address is being protected from spambots.
