Player Application and Registration

Player Registration is Open.

A Multi-Step Process for Fall 2013 & Spring 2014

Returning players please mail your player card, or fill out the card online.

Step 1: Player Card - Go to cottondew.com, Complete a Player Card for each player in your family. (You should get a confirmation email.)

Step 2: eAYSO Player Application - Go to eayso.org  Login to your existing account or, if you are new to AYSO, create an eAYSO account. Complete a Player Registration form for each player in your family and e-Sign each application. Print out one copy of the completed application form each. Step 3: Submit All 2 Items - (a) each player’s copy of their e-signed Player Registration form, and (b) a check or money order for the fee ($65) to AYSO. Submit the forms & fees by mail to: AYSO Registrar, PO Box 516, Philomath, OR 97370. (I you have all of the information you can drop it off on Saturday at the AYSO HQ)

□ Payment (Write a Check to AYSO for the $65 Fee 1 check per child)

□ Turn in (Mail everything, except player card, to AYSO Registrar PO Box 516, Philomath, OR 97370)
