Registration and Application Low Down

What is Needed for Fall 2015 Season

  • Print only 1 copy of the electronically signed Player Pre-Registration form
  • (New Players Only) Visual proof of age/birth date. (we do not keep this proof)
  • Proof of Paid Registration Fee ($80) to AYSO (receipt at event, or online receipt from eAYSO credit card)

Complete Registration at Registration Event with registration volunteers at Adams Elementary School on the dates in the events to the right. (if none, contact the registrar)

The eAYSO legal forms are required to be considered for play. A $80 fee, paid to AYSO, is required for each player.*

  • eAYSO is the system we use for pre-registration/application/liability waivers/legal forms. Without this form you are not an AYSO player.
  • Normal Fee of $80, paid to AYSO, is required to play. The majority of this fee goes to insurance and uniform. Without the fee you cannot play.*
  • For new players, we need to verify the players birth date, please provide a paper document that will prove the players birth so we can verify.

*Scholarship to reduce the fee to $35, paid to AYSO, can be applied to qualified applicants. A qualified applicant is a family that has been accepted into the meal program. You must provide a copy of your letter from the school district showing that you receive free or reduced lunch for this school year. Proof of this acceptance needs to be provided, or you will be required to pay the full fee.

Fall 2015 Registration Instructions

Registration Fee: $80.00. Please make checks payable to AYSO, and write a separate check for each child.

How to Register:

  1. Log on to eAYSO using the same email address and password you have previously used.  On your home page, select "Application for a returning player." Update any information as needed.  Please do not create a new account. If you have trouble logging in to your account in eayso, please click on the link for password or email help on the sign in page. If you continue to have difficulties please contact (here) and provide parent name and player’s name.
  2. Bring the following to the registration volunteers in Adams gym between 9 AM & 2 PM on May 2 or May 9.
    • 1 copy of the electronically signed Player Pre-Registration form (from eAYSO)
    • Payment
      • Check made out to AYSO or cash
      • If you wish to pay by credit an option will be available to you at the end of your online registration. If you pay online, please print your receipt and bring this to the registration event.
    • Copy of your child’s birth certificate for new players only (if your child has played in our region, we do not need a copy of their birth certificate.) We do not keep this copy. It is for age verification purposes only and will be returned to you after your registration.

Practice Areas: There are three practice areas from which to choose. The practice areas are geographic north, central and south. Specific practice days and locations will continue to be determined by the individual coaches and teams as has been done historically.

Special Requests: If your child has special needs, please make a note for the team formation committee by filling out this form (here). Please be sure to include a brief description of the request and reason. Children are divided into divisions by their age as on 7/31/15, not by grade. AYSO National rules do not allow a child to play in a division below their age. Children with August birthdays who are entering grades 1, 3, 5, and 7 should request to move up to the next division if they would like to play with their classmates. Siblings who are in the same division may play together on the same team. Please indicate siblings name(s) on the pre-registration form in the box indicated. For safety reasons, we cannot move younger players up divisions just so they can play with a sibling in an older division.

Refunds: To receive a full refund, your request must be received before teams are formed in mid June.  Exceptions are granted if you move out of the region or your child is unable to play due to a medical reason and you request the refund before Sept. 1,2015.  Please provide a self-addressed stamped envelope and mail your request to: AYSO Registrar, PO Box 516, Philomath, OR 97370.

AYSO Insurance: All registered AYSO players, coaches, referees, and other volunteers are covered by Soccer Accident Insurance.  Brochures that provide a summary of coverage are available at the Information/Safety Station.

Adult Volunteers: AYSO National requires all Board Members, Coaches, Assistant Coaches, Referees and Team Managers to electronically submit a new Volunteer Application form each membership year. Please log in to, follow the links to the volunteer section, create or update your information and e-sign. Returning volunteers no longer need to show proof of ID every year! But all new Volunteer Application forms must be submitted in person, with photo ID to the region’s Child & Volunteer Protection Advocate (CVPA). Upon completion, print 1 copy, and bring it to the registration event listed in the calendar.

Application Times

 We run a member year, the member year is exactly the same as a school year in our area.

Three types of registrations:
  • Open Application in May registers for both Fall and Spring.
    • New players includes at least two registration events.
    • Returning players includes a team registration period where they submit a registration packet with the team.
  • Limited Application in August for both Fall and Spring and in February for just Spring.
    • All applicants are considered new, this registration is only to fill vacancies on teams.
  • Deadline Application before first game.
    • All applicants are considered new, this registration is only to fill a team NEED, and it is usually conducted with the instruction of team formation.

Refunds - NO REFUNDS will be issued to players that are placed on a team.  If we are unable to place your child on a team, a full refund will be issued.
